Would you get medical treatment at your school's hospital/clinic?

This is a pretty difficult question. It sounds from your description that any local specialist care would have to be through the health system that your school is in. If that is the case, the two things I would weigh are first whether or not you can commit the time to drive an hour multiple times a month (at least initially) or second some inevitable loss of privacy. I cannot decide that for you. Depending on the size of your local system, you may completely avoid having your providers be your preceptors, but you probably will see at least some of them around the hospital. My wife was diagnosed with an autoimmune disease early in med school and my sons are autistic, but we chose to stay in the same system as my medical school. The drive times would have been prohibitive, and my and my wife's needs for privacy were not so great compared to huge benefits of single system, local care. That said, I am now a 4th year and have avoided any preceptor relationships to this point. I will do a developmental peds elective before graduation that will bring me in direct contact with the pediatrician who diagnosed my sons, but I've already made sure there were no concerns from him, the administration, or on my own part. In the end you need to decide this for you. I would personally lean toward local care as you approach Step 1 (or COMLEX), as your schedule will shrink dramatically. Also, if anything should go wrong, all your records would be local. You need to take a hard look at your needs and worst case scenarios and make the best decision you can. You will inevitably run into unanticipated problems, but a good plan make them less common and more manageable.

On a separate note, you will probably need to approach your administration eventually. If you are healthy and clear headed, you have time, but it is better in my opinion to demonstrate good grades while working on the diagnosis than to have to explain away any issues (from schedules to grades and all points in between) when they did not even know you have a problem.
