Not late at all if it's your freshman year. I don't really understand though what you meant by no transportation -- did you mean no transportation for volunteering or in the city in general?
Consider both what you are interested in and what your school values for pre-meda (e.g. some schools require letter of rec from a research professor or principal investigator) and schedule your semester and semesters to come around those things. It might help to talk to a pre-med advisor or advisor in your major.
